Call (419) 925-4613.

We suggest you call our office to make an appointment to avoid excessive waiting for you and others. For any reason, if you are not able to keep an appointment, please inform us as early as you can.

We suggest that you call us to schedule an appointment 2-3 days in advance so that we may be able to accommodate your schedule

Can I have a Wellness exam when I am sick?

No. Your presenting complain becomes important in determining whether you are seen and billed as a Wellness/Preventative Medicine exam( physical, pap etc) or as a problem oriented exam ( cough, sore throat etc) Please note that these are two distinct type of visits. We can not do justice to your medical needs if we try to roll preventative care together with problem solving and we are required to bill for each separately, using mutually exclusive codes. We understand that this can be frustrating, but we have no control over insurance regulations.

Why do I need to be seen every 6 months for medication refills?

You are on medications because you have been diagnosed with a disease. Our physicians need to monitor the disease process and your response to the medications you are currently taking. Please keep in mind to arrange an appointment one month prior to running out of medication. If you need a prescription called in because you do not have enough medication, there is a $15 fee. .

Why can’t I make an appointment for tomorrow when I am sick?

We have Same Day Scheduling. This means that there is a large block of appointments reserved everyday to accommodate our acutely ill or newly injured patients. They can not be scheduled in advance; you must call at first thing in the morning to reserve your appointment. Our phones open at 7:30 am every day, so please call early.

What hospital can I go to for admissions, laboratory or x-rays?

You can go to any hospital of your choice. We have privileges at both Mercer Health and Joint Township District Memorial Hospital.

What payment types are accepted?

We proudly accept Visa, Master Card, checks and cash. A valid driver’s license or an official Ohio picture ID is required for all checks.
